How Fotona TwinLight Periodontal Treatment Works

TwinLight Periodontal Treatment (TPT) uses two laser wavelengths in sequence — each one matched to what that tissue type actually needs. That's what separates it from single-wavelength laser dentistry and from conventional periodontal treatment. The combination of Er:YAG and Nd:YAG in a single protocol allows each step to be clinically optimized rather than handled with one instrument doing multiple jobs.

The entire procedure is performed without a scalpel. No cutting, no sutures afterward.

Nd:YAG Laser

Remove diseased tissue from the pocket lining

The Nd:YAG laser removes the infected epithelial lining inside the periodontal pocket and improves access to the root surface below. This is the tissue responsible for maintaining the infection — removing it stops the active disease process and prepares the root for cleaning.

Er:YAG Laser

Clean the root surface

The Er:YAG laser is used to remove bacterial biofilm, calculus (tartar hardened onto the root), and smear layer from the root surface. This is the key step — cleaning the root is what allows the gum tissue to reattach and the pocket to close. The laser does this more precisely and less invasively than hand instruments alone.

Nd:YAG Laser

Promote stable healing

The Nd:YAG laser is used a second time to coagulate the treatment site and promote the formation of a stable fibrin clot — the foundation for tissue reattachment and healing. This step also provides additional deep disinfection of the surrounding tissue.

What to Expect from Your Appointment

TwinLight Periodontal Treatment is performed in-office with local anesthesia to keep you comfortable. The procedure fits into a standard dental appointment and doesn't require general anesthesia or sedation.

During treatment, the laser handpiece is used at the gumline and inside the periodontal pocket. There's no cutting sensation and typically no bleeding. Most patients describe it as noticeably different from what they expected — quieter and less invasive than prior dental experiences.

After treatment, there's no suture removal because there are no sutures. Most patients return to normal activity the same day. Some mild sensitivity along the treated gumline for the first few days is normal.

In the weeks that follow, the gum tissue gradually reattaches to the cleaned root surface, pocket depths reduce, and the tissue firms up. Healing continues over several weeks. Dr. Dontsova will schedule a follow-up to measure pocket depths and assess the tissue response.

Gum disease requires ongoing maintenance, not a one-time fix. Dr. Dontsova will put a clear maintenance schedule in place — typically more frequent cleanings for the first year, then moving to a stable interval based on your response.

Who Is a Good Candidate for Laser Gum Treatment?

TwinLight Periodontal Treatment is appropriate for patients with moderate to severe gum disease — cases where standard cleanings haven't resolved the infection, pockets are deeper than 4mm, or the condition has been progressing despite treatment.

Is laser gum treatment painful?

Local anesthesia is used throughout, so the procedure itself is not painful. Most patients are surprised by how calm the experience is compared to what they imagined — there's no cutting sensation, no suture placement, and no moment where something uncomfortable happens that they're bracing for. Post-procedure soreness is typically mild and resolves within a few days. Over-the-counter pain relief is usually sufficient.

How many treatment sessions are needed?

This depends on the extent of the disease and how many quadrants of the mouth are affected. Many patients are treated in two to four sessions — often completing two quadrants per appointment. Dr. Dontsova will map out the full treatment plan at your consultation. Maintenance appointments after active treatment are also important and will be scheduled accordingly.

Will my insurance cover laser gum treatment?

Can gum disease come back after treatment?

The bacterial environment in the mouth is ongoing, and some patients are genetically more susceptible to periodontal infection. That's why the maintenance schedule after treatment matters. Regular periodontal maintenance appointments, more frequent than standard cleanings, are what keep the disease controlled long-term. Dr. Dontsova will give you a clear plan.

Is this different from a "deep cleaning"?

Yes. A deep cleaning (scaling and root planing) uses hand instruments and ultrasonic scalers to clean below the gumline. It's appropriate for earlier-stage disease with pockets under about 5–6mm and no active surgical indication. TwinLight Periodontal Treatment goes further: it removes diseased tissue from the pocket lining and promotes healing with the laser in addition to cleaning the root surface. It's used when deep cleaning hasn't resolved the problem, when pockets are deeper, or when the disease is more advanced.

How long does recovery take?

Most patients return to their normal routine the same day or the next morning. There are no sutures to heal, no incision sites, and no need for a soft diet beyond personal comfort. Some sensitivity along the gumline for a few days is normal. The tissue healing process continues over several weeks as the gum tissue reattaches and the pocket depths reduce — you won't feel that part happening.
